Both.

Usually when you start to get gout it will present as an acute attack. But if you leave it to get worse then you can end up in a situation where the attacks come so frequently that there is not enough time between them for the body to heal.

When that happens you will have a chronic gout problem.

And take it from my personal experience. You don't ever want to go there!
