View Full Version : Vitamin B12 injection brings gout attack

I have been on Allupurinol 300 mg perday for the past 3 months. It has lessen the frequency of my gout attack.
I have been treated with vitamin b12 monthly injection for the past 15 years due to vitamin b 12 deficiency with neurologic symptoms.
The problem is I get gout attack 1 day after each time the vitamin b12 shot is given .

I can't understand why ? Should I take colchicine as a profolaxis 1 day before giving my B12 shot ???

I do remember the hematologist who gave me the first B12 shot said that there are two side effects of vitamin b12 shot :
1. it may bring gout attack.

2. lower the potassium level.

I know that my gout is caused by high uric acid ( 8.6 ) due to over production . urine uric acid 24 hr test also showed high level .
I just don't understand why B12 shot can induce gout attack ?

Also heard that Thyroid supplement for hypothyroid can also bring gout attack .

I used Pure Vegan B12 spray.There is also a cheaper version that does not advertise vegan but says so on the label called Pure Advantage B12. The ingredients are identical.

I think you should not take colchicine because a few "Cyanocobalamin [Vitamin B12] injection prescribing information" state: "Colchicine, aminosalicylic acid and its salts, and excessive alcohol intake lasting >2 weeks may reduce absorption of vitamin B12 from the GI tract." How about asking your pharmacists or doctors what you can take for gout flares instead.

The facts that your blood uric acid (UA) is 8.6 and the 24-hr urine UA is high, indicate: Allo 300 mg/d is not strong enough for you. You may like to discuss with your doctor about increasing Allo dosage (maximum= 900 mg/day), or replace Allo with another med, e.g., Uloric, so your 24-hr urine UA is in normal range and your blood UA level is < 5~6 mg/dL.

In the meantime you need to prevent UA kidney stones from forming by alkalizing your urine and drink sufficient water to dilute the urine to light yellow. Talk to your doctor about UA kidney stone prevention, too.

Sorry, I just added the most important aspect to item 14):
Take 200 micrograms folic acid the day before, during and after the shot.

Gout is caused by vit B12 shots mostly when vit B12 is much high than folic acid. Avoid booze the day before, during and after the shot, which increases elimination of folate
