knowledgeable than me, and right now that's everyone on here I'm sure!<br />
<br />
 I was recently diagnosed with gout when my ortho doctor drained my knee and found the fluid was positive for the uric acid crystals. My knee still swells a bit (3 weeks later) after activity, but i'm on NSAID's and so no pain, just some stiffness.<br />
<br />
 I think from talking to a rheumatologist that the attack occured because I got off of HCTZ, which made my uric acid level change, and even tho it would help the level, it apparently changed the level and that can trigger an attack.<br />
<br />
 Looking back i have had some foot/ankle problems the last few years that my have been gout and i just didn't know it. All of which were after I got on the HCTZ (starting within a year of getting on it.)<br />
<br />
 My questions are the following:<br />

 Do people think getting off the HCTZ is enough, or now that i've had an attack, should I get on medications for it?<br />
<br />
 If I should take something, should i wait until the knee no longer swells, or get on quickly?<br />
<br />
 I ask these things because I did go to a rheumatologist who prescribed allopurin (sp?) at 300 mg after my story, but there was no bloodwork or anything else that was done other than me telling him about the orthopedists findings, and that just seemed weird, and I'd hate to get on something that could trigger another attack(s) and that i'd have to stay on forever...<br />

				OBJECTIVE:<br />
As many as half of all patients with gouty arthritis have some degree of renal impairment. The goal of this systematic review is to provide physicians with a comprehensive examination of available data on the risks and benefits of gouty arthritis treatment options when used in patients with chronic kidney disease (CKD).<br />
<br />
METHODS:<br />
We conducted a systematic literature review to determine what information is available to guide treatment decisions in this patient population. PubMed was searched for English-language articles indexed through July 2011 containing the terms &quot;gout&quot; or &quot;hyperuricemia&quot; and synonyms for renal impairment in combination with drug names. Publications were deemed relevant if they reported results from clinical studies, case reports, or prescribing practices of the drug of interest in patients with gouty arthritis and CKD.<br />
<br />
RESULTS:<br />
Nonsteroidal anti-inflammatory drugs and colchicine are oftentimes not considered appropriate in patients with CKD. Corticosteroids may be an effective alternative in this population; however, their efficacy has not been confirmed in randomized controlled trials and these agents can cause serious side effects. Allopurinol can be used for the prophylactic management of chronic hyperuricemia in patients with CKD, but the recommended decreased dosage may limit efficacy and serious hypersensitivity reactions may preclude its use. Febuxostat and pegloticase are new treatment options for chronic urate-lowering prophylaxis; however, the safety of these drugs in patients with advanced CKD has not yet been reported.<br />
<br />
CONCLUSIONS:<br />
There is currently an unmet need for additional treatment options for the management of gouty arthritis in patients with CKD.

For the past decade I have been subjected to 3 or 4 gout attacks a year.  They have been in my right big toe, left second toe and right second toe.  I will not describe the debilitating pain and effects because we all now that too well.<br />
<br />
During my decade I have simply relied on pure cherry juice when symptoms begin to show. And that worked to shorten the bouts, even though I was sitting on the john half my day.  But, over the decade my entire toes on both feet had become completely stiff and rigid and there was a constant low level pain.<br />
<br />
I finally moved to a more aggressive approach and my cure when the early symptoms started to show in my ankles and knees.  There was no way I was going to get gout in my knees.<br />
<br />
I did research and have found an incredible natural method that has me literally jumping and running.<br />
<br />
Four days ago I started taking 1000mg of Quercetin and a pinch of natural celery seeds in the morning.<br />
<br />
I am not prone to hyperbole, but I sit here tonight...and feel compelled to share this news...because I am absolutely wiggling my toes with complete neural sensation for the first time in a decade.  I have strength in my toes, I can totally feel my toes and I have dexterity in my toes that have been missing for ten years.  In four days.<br />
<br />
The only side effect is that I have urinated what seems like 1,000 times during the last four days...but that is overwhelmed by my walking non stop just feeling how great my feet feel.  I can't stop walking and lifting up on my toes.  It is amazing and I hope it lasts.</div>

My new doctor just started me on Uloric 80 mg. one week ago. I seem to be having mini gout-flare pain in all the joints where I have had serious gout flares in the past. Today I began having pain in the area of both kidneys. I've read much about the pain that comes with starting Uloric due to dissolving crystals. Anyone else heard of kidney pain associated with this transition?</div>

Diagnosed with Gout around age 20 so very young. In fact the first case my doctor had seen in someone that age.<br />

I put it down to poor diet and booze. Attacks for the first five or so years would only be once and max twice a year. And they would last a few days tops so i would take normal pain killers and that was the end of that.<br />
<br />
As i hit 30 they would happen three to four times a year. Then it got to the point i was just living with the pain everyday. We are talking big toe and both knees at the same time it was crazy!<br />
So after one really bad night of pain i started taking the Allopurinol everyday not just for a few weeks but everyday and it's almost been two years now and i have never had an attack since.<br />
<br />
My diet as got better as in i don't eat as much red meat or drink beer but we are not talking drastic changes here. For me the key was to buckle up and own it not the other way round. It's about keeping down the uric acid levels and allopurinol is the best thing to deal with that. Once you get past the first few months the allopurinol will start to dissolve the crystals that form also.<br />
<br />
It's not going to work for everyone but i can only go off what allopurinol everyday as done for me. I still take it now just to keep my uric acid levels down , the last time i went the docs the test came back that my levels are way under and the crystals have gone in my foot.<br />
My right toe still bends in because i did nothing for years and that will never change but that's my fault for not taking action when i should of.<br />
Don't be a hero if you have Gout just start working on way today to get this evil out of your life.<br />
Nobody as to live with that pain it's to much!</div>

And the main reason that I'm posting now is that I'm just so frustrated with this damn &quot;never ending it seems gout&quot; that I just had to vent somewhat. Wish we all had the &quot;silver bullet&quot; for this disease but we don't and unless you have it, you sure don't understand it. Another reason I'm posting here. <br />

My history: I'm 45, 5' 11&quot; tall and weigh about 195 lbs. In pretty good shape for 45 I think anyway. Don't hit the gym much these days but do push-ups, pull-ups, walk and have a active lifestyle outside of office job. I eat OK and avoid fast foods, try to eat fruit fairly often but I do like my beer on the weekends. <br />
<br />
I have been experiencing gout since my mid to late 30's. Couple of times a year for several years, sometimes go to the doctor and get on meds to prevent and to treat the flares if bad enough. Even had both of my knees drained of the fluids on them this past summer of '11 during a really bad attack. After that attack on the knees I didn't have another one (severe anyway) until winter of '12 and from then on to present it seems I have been dealing some sort of flare or attack all year long. Only change in my habits as been some financial stress and I have zero clue if that has anything to do with it.<br />
<br />
Like I said, it use to happen much less and then only a joint at the time. Right now I have it in my left foot, my left knee, my left pinkie knuckle and at the base of my right wrist. The pain in all of the joints are probably around a 4 or 5 on a scale of 10 (10 being highest) right now. Just took some Advil is all. Drinking my water like I always do as well. I don't drink cokes or much tea much. <br />
<br />
I am just so frustrated with this aggravation of this continuous cycle almost weekly now and with multiple joints too boot. My kids are beginning to really worry about me when each morning I get up and have to hobble each day to take them to school. I am widowed and have been for 5 years now but have been dating a great girl recently but she doesn't know anything about gout and sometimes looks at me like &quot;oh lord, what's this guy going to be like in 5 or 10 years if he's like this now?&quot; <br />
<br />
Again, sorry for the long rant but thanks for reading and any advice would be appreciated. Going to make an appointment to see a doctor and I guess get on something to help. Oh and I forgot to say that about 3 or 4 years ago and I had the most viscous of attacks in my right elbow and now it looks like a witches elbow or something. It's kinda of pointed now instead of being round. Man I hate gout!!!!<br />

Just this past week, I had an outbreak in my wrist. I happen to be one of the lucky ones who rarely gets the pain in the toe. My hand/wrist/arm swelled from my finger tips to half-way to the elbow. I can finally use it enough to type today. Although that &quot;healing nerve pain&quot; is getting me this morning. I was so bummed out yesterday when I woke up to the hand still hurting, and a new outbreak happening to the top of my foot. Never had two at once. I though, you have to find some support from from other sufferers, or you need to shoot yourself. So here I am. Misery loves company, eh? I feel for everyone on this forum. I wish you health:)</div>

However both my father and my Husband of 26 years do have it.:( I guess hubby's story begins the morning he woke me up telling me something was very wrong and I needed to take him to the hospital. I figured he must be dying for him to agree to see the doctor!:eek: Turned out to be kidney stones (which appear to be related to gout). Took a week for that to resolve.This was his first major illness. His only health problems before were injuries (ie bad knee from basket ball.) <br />
<br />
About a year later his foot began to hurt. Gout was my first thought but he wasn't convinced (and wouldn't go to the dr). By the time it moved to the other big toe I had him convinced, though. He spent a month off his feet. he has had minor flare ups in the year since then. We just have him take some herbs (Go-out= cherry, pineapple, celery, tumeric) and keep staying hydrated. <br />
<br />
About a week ago he came home limping (not the normal knee-limp). Again, at first he thought it was an injury to his ankle. I knew better and this has been a bad attack. I have him taking valerian and cat's claw and I rub an herbal salve I make into it (Aloe, comfrey, plantain, olive oil, beeswax), but it has been very slow to get the swelling to go down. I will be telling him about fructose tonight. He is addicted to Coke Cola so that makes sense. <br />
<br />
We are total tea-totalers, don't eat organ meats, don't smoke, etc. But his dad had gout:( We wish he did drink or eat organ meats so we would have more things we could try to stop this.:rolleyes: <br />
<br />
Besides wanting info for hubby and my dad, I have nine children, four of them boys. With two grandpas, a dad, and an aunt with gout, I am afraid it is inevitable for them to get it. The more information i have to share with them the better.</div>

My Dr gave me auprolnol and I'm taking it but I keep reading that gout might mean a much more serious condition. Is this always the case or does gout just affect some people. I am obese but the last 9 weeks have switched my diet and been in the gym...I have been eating a lot of oatmeal and I read that it can cause gout. Any help would be much appreciated.</div>
